A Link to Care works with an ever expanding list of physicians and care providers led by Dr. Edmond Derderyan. Derderyan medical group offers mobile PCP services by good, knowledgable and caring healthcare providers.

There are times when we as a community can make decision on our own, we only need information regarding insurance and the answer to "How to" or "What if" questions. While we want to be a referral source, guide and build a relationship with you, we first and foremost want to help our community be knowledgable. We are an information resource, let us answer your questions!

One of the most difficult realities of life is the transition from home living to out of home living for the health benefit of an individual. Understanding that aspect,  A Link to Care aims to ease the stress and anxiety of the process by providing a caring and knowledgable support system to walk you through the process. Through our knowledge, relationships and concern for good healthcare we are prepared to make great referrals to the best out of home placement for you and your families needs.

 

As a relationship building staff our goal is to serve as a resource from beginning to end. Including; out of Home placement referral, continued support from time of admission, discharge and relocation.
